Community Team

The Hospice's team of Clinical Nurse Specialists (CNS) visit patients in their own homes. Often, their visit is the first contact that a patient has with Phyllis Tuckwell Hospice, so they try to ensure that questions are answered and any fears or anxieties are allayed. The CNSs form a vital link between the comforts and reassuring surroundings of a patient’s home and the full range of clinical expertise that exists within the Hospice. They address not only physical symptoms, but also emotional and psychological problems faced by patients and their carers at a very stressful time.

We recognise the need to develop our Community Team further, to enable us to see more patients at home and work even closer with more local healthcare professionals to ensure our community is well-served by palliative care specialists. The Community Team spend a lot of time educating our local healthcare partners (District Nurses, GPs, nursing homes, etc.) through a formal training programme managed by the Hospice. Our aim is to make sure everyone is West Surrey and North East Hampshire has access to the palliative care they need when they need it.
