Patient welfare team
The Patient Welfare Team provide advice, information and assistance in financial and legal issues. The team advise on matters such as claiming welfare benefits, utilising private healthcare insurance or dealing with debts. They will also advise and assist with legal issues, such as drawing up an enduring power of attorney or making a will and in practical matters such as planning a funeral.
The Social Workers provide information and support on a practical and emotional level to In-Patients, Day Hospice patients, patients at home and to their relatives and carers. They work closely with the nurses, doctors and other members of the multi-professional team to ensure that there are smooth discharge arrangements for patients who may wish to go home from the Hospice. Where discharge is not possible, they will provide practical information with regard to the application to nursing homes and will assist relatives and carers in applying for funding if necessary.
