Social Work Support

The Hospice has its own Social Work Team who work with the local Social Services departments and the NHS to ensure anyone who requires support is able to access it. This could be care in your own home or, if you can no longer manage, a nursing home. 

Working out who to contact or where to access funding can be confusing when there is so much going on, and the social work team are here to help.
